Your First Visit

At Quality Physical Therapy, providing physical therapy in Burlington, WA and Skagit County, your care and comfort are our priority from the moment you step into our clinic at 1838 South Burlington Boulevard.

During your visit you can expect to receive a thorough evaluation so that we can determine the root cause of your pain and create a customized treatment plan to get you back to your normal level of activity as quickly as possible.

If time permits, you may receive some physical therapy at this first visit, so please wear comfortable, loose-fitting clothing.

Please bring with you a full list of any prescription or over-the-counter medication you take regularly, as well as any notes or prescription from your doctor.

If you can, please print the forms on our website and complete them prior to your arrival to expedite the paper work.

We look forward to easing your pain and helping restore your quality of active living.



Quality Physical Therapy accepts most major insurance providers.

If you are not sure about your coverage, please don’t hesitate to call our office in advance of your appointment. The number is 360-757-9335.

Washington is a direct access state, so you can see a physical therapist without a prescription from your doctor, but please be aware that most insurance providers do insist on having a prescription for our services.



Registration & Consent Forms

For your convenience you can download the patient registration form. You will need these completed prior to your first visit.

If you are unable to do this prior to your appointment, please arrive at least twenty minutes early to fill out the forms.



Monday to Thursday: 8 a.m. to 5 p.m.

Friday: 8 a.m. to 12 noon

Closed Saturday and Sunday
