Premera, Blue Cross, Regence, Cigna, Conneticut General, Farmers, Medicare, First Choice Health, Labor & Industries, Teamsters, TriCare, Trusteed Plan, Uniform Medical, First Health Network, AARP, Great West Health Care.
Due to the large quantity of insurances not all are listed above.
Insurances out of network but excepted:
Aetna, Group Health Options, Group Health Alliance Plus, United Health Care, Pacific Care.
Insurance Plans that we do not accept:
WA Basic Health, Community Health Plan, D.S.H.S, Medicaid, Pacific Care Secure Horizon, Molena Healthy Options, Regence Healthy Options.
The Steps to Scheduling your Appointment:
Insurance Confirmation
Prior to booking your appointment you will want to verify that your insurance allows our facility as an accepted provider, or what information is required prior to treatment in order for them to pay for outpatient physical therapy. Our receptionist will call on your behalf to check your benefits at your first visit, however calling prior to this will avoid an unnecessary cost charged to you.
We offer discounts for clients that would like to pay cash or credit at the time of service who do not have insurance plans.
Doctors Prescription & Insurance Card
You will want to bring in your doctors prescription and insurance card at your initial visit. If you do not have either one you will need to let us know prior to your arrival.
Registration & Consent Forms
For your convenience you can download the patient registration, condition and consent forms. You may fax them prior to your appointment or bring them in on your first visit.
If you are unable to do this prior to your appointment, please arrive at least fifteen minutes early to fill out the forms.
Scheduling an Appointment
You may contact us by phone, fax, or internet. We do our best to accommodate you as expediently as possible. Typically we are able to arrange a time within 24 hours.
Appointment Day
Please arrive with enough time to give the receptionist any necessary paper work such as doctors prescription, insurance card, registration forms, MRI, or X-ray information.
Dress comfortably with loose fitting clothes for a complete assessment.
Anticipate the session to last 40 minutes. Your therapist will conduct an assessment to determine the best course of treatment. Following the exam she will review with you an at-home exercise routine which suites your needs.
